Fill in a register a civil partnership and apply for a certificate application form
Use this application form to register your relationship as a civil partnership without a declaration ceremony.
You need to be 18 or older, and at least 1 partner must live in Queensland. Neither partner can be married or already in a civil partnership.
Step 1—Complete this part online
- Start the form by answering the question below—it should take you less than 15 minutes.

- Review your details.
- Read the email we send you to find out how to finalise you application.
- Download and print the completed application form.
Step 2—Complete on paper and submit
- Gather your proof of ID and supporting documents.
- Visit a Justice of the Peace with your partner. Take your proof of ID and supporting documents to have them certified and to complete the statutory declaration section of the application form (if submitting by post).
Proof of ID documents must be submitted with your signed application. When submitting by post, have your documents certified by a Justice of the Peace or another authorised person.

Rules for registering a civil partnership
To register a civil partnership you must:
- be over the age of 18
- not be married or already in a civil partnership (or registered relationship)
- not be in a prohibited relationship
- have at least 1 person in the relationship living in Queensland
- provide evidence that you are no longer married (such as a divorce or death certificate) if you have ever been married
- both read about the legal effect of a civil partnership.
